Do not send personal health information through general email or an unsecured website form unless Motiva specifically directs you to an approved secure clinical system.

Personal health information used for clinical care should be collected, stored and managed through the clinic’s approved systems and processes rather than ordinary website pages. Clinical privacy practices may be governed by additional policies provided when care begins.

This website may link to external services such as maps, booking software or social platforms. Those services have their own privacy practices and policies.

When you join the Motiva Opening List, you may provide contact information such as your name, email address and any other optional contact details requested by the signup form. Motiva uses this information to send the clinic opening updates and appointment-availability information you requested.

The Opening List is provided through Brevo, Motiva’s email communication provider. Information submitted through that form may be processed and stored by Brevo according to its privacy practices. Please do not include symptoms, diagnoses, medical history or other personal health information in the Opening List.

Submitting the Opening List form means you are asking Motiva to send you opening and appointment-availability emails. You can withdraw that consent at any time by using the unsubscribe option included in Motiva emails or by contacting info@motivaphysiotherapy.ca.

Motiva will update this privacy notice as its booking, clinical and communication systems are finalized.
